For those wanting checkboxes and free text fields, I used a real-word template for this from a service provider in an entirely different industry — to be representative of how these types of forms often are designed.
Also, the "Cancel" button on a "Cancel" form is on purpose ;)
To be clear: I did not have any concern for making this form user-friendly. That would be giving the providers too much credit. 😅
