Mullvad is actually the only one I trust on this list. In their case it's possible, for purposes of anonymity, to send cash payments through the post.
The only thing you need for their service is a generated account number. They don't ask for name, email, password, anything… and no subscriptions.

If mailing cash is not your thing you can also buy physical voucher cards. This keeps third parties from being able to link a payment to a Mullvad account.
In Sweden you can also buy these at Webhallen (physical electronics store) using cash.
